Greetings new and incoming patients!  We are so happy you’ve decided to pursue therapy for snoring or sleep apnea.  To provide you with timely and quality care, our sleep dentists, Doctors Emerick and Canfield, have provided forms to help your office visit go smoothly. Before visiting our office at 450 Alkyre Run Drive Suite 300, you can fill out these forms to help reduce office time. Who doesn’t like reduced office time?

 

Please bring the completed forms to your free initial consult, in addition to the following information:

  • A copy of your most recent sleep study
  • Your insurance information

If you do not have a printer or support the green initiative, we encourage you to call the office prior to your visit and we will happily send you an electronic link with the necessary paperwork to complete prior to the visit.
